Community Support

By addressing these critical needs, Community Supports can prevent the need for more expensive and intensive care. Members who qualify and whose plans find the service both medically appropriate and cost-effective can access these alternative services, ensuring they receive the care that best suits their needs.

Housing:

•  Housing Deposits: This service supports members by covering key housing expenses such as security deposits, utility setup fees, and initial rent payments. Additionally, it provides funding for essential medical items like air conditioners, heaters, and hospital beds, ensuring the home is safe and ready for move-in.

•  Housing Tenancy and Sustaining Services: Once housing is secured, members receive ongoing support to maintain their residency. This includes working with landlords to resolve any issues, assisting with the annual housing recertification, and connecting members to community resources that help prevent eviction and ensure long-term housing stability.

Respite Services:


Respite services provide short-term relief for caregivers of members. These services can be provided either in the member’s home or at an approved facility. Caregivers can receive support on an hourly, daily, or overnight basis, depending on the needs of the member.

 

Personal Care and Homemaker Services:

 

Members who require assistance with Activities of Daily Living or Instrumental Activities of Daily Living receive in-home support such as bathing or feeding, meal preparation, grocery shopping, and accompaniment to medical appointments.

 

Medically-Supportive Food/Medically Tailored Meals.

Members receive deliveries of nutritious, ready-to-eat meals and fresh groceries tailored to support their health needs. Additionally, they are provided with vouchers for healthy food options and access to nutrition education.

ECM Includes:

Coordinating care and integrating service
Linking members to the community resources
Addressing social drivers of health, such as environment, education and access to quality health care, to improve health outcomes

ECM Eligibility:

Adults with high health care needs/High utilizers with frequent hospital or ER admissions
Adults with serious mental illness (SMI) or substance use disorder (SUD)
Adults living in the community and at risk for LTC institutionalization
