What is ECM
Bullet Points
Enhanced Care Management (ECM) is more than a Medi-Cal benefit—it’s a lifeline for those in greatest need. By building strong, trusting relationships, care managers provide essential support, whether on the streets or in the comfort of a member’s home. ECM doesn’t just address medical issues; it connects members to vital resources like food and housing, ensuring that every aspect of their well-being is considered. This program is a testament to the power of compassionate care and comprehensive support.

Community Support
Community Supports are transformative services offered by Medi-Cal managed care plans, targeting the social determinants of health to help members lead healthier lives. By addressing these critical needs, Community Supports can prevent the need for more expensive and intensive care. Members who qualify and whose plans find the service both medically appropriate and cost-effective can access these alternative services, ensuring they receive the care that best suits their needs.
